Early Intervention Service Coordinator

Community Options, Inc. (COI) is seeking a dedicated Early Intervention Service Coordinator to support infants and toddlers with developmental delays and their families-primarily in the Gunnison/Crested Butte area.

This is a highly rewarding role for someone who is organized, self-motivated, and passionate about helping others navigate complex systems of care.

What to Expect in This Role

This position combines relationship-building, service coordination, and compliance-driven case management.

  • You will manage an active caseload and coordinate multiple services and providers
  • Much of your work will be independent and self-directed, including scheduling and travel
  • You will balance family support with state-required timelines, documentation, and reporting
  • Regular local travel is required (primarily day trips)

This role is ideal for someone who thrives in a fast-paced environment, can manage competing priorities, and is comfortable working both independently and collaboratively.

Key Responsibilities

  • Coordinate and monitor early intervention services in accordance with Individualized Family Service Plans (IFSPs)
  • Facilitate IFSP meetings and collaborate with families and service providers
  • Ensure compliance with state timelines, documentation, and reporting requirements
  • Support families in understanding their rights, services, and available resources
  • Maintain regular contact with families and document all interactions
  • Coordinate evaluations, referrals, and transitions to preschool services

What We're Looking For

  • Bachelor's degree OR equivalent combination of education and experience (minimum 4 years) in a related field
  • Valid Colorado driver's license and acceptable driving record
  • Ability to pass a criminal background check
  • Strong organizational, time management, and documentation skills
  • Ability to work independently, manage a caseload, and meet deadlines
  • Strong interpersonal and communication skills
  • Spanish-speaking skills a plus
